YERMO — Silver Valley Unified School District is taking a major step toward supporting the whole family with the launch of a new weekly health clinic, offered in partnership with A Greater Hope.

Beginning this month, the Enhanced Care Management and Community Supports (ECM/CS) program will be available every Friday from 8:00 AM to 12:00 PM at the Silver Valley School-Based Health Center, located next to Yermo School.

The service is a free Medi-Cal benefit designed to help eligible individuals and families with complex medical and social needs connect with coordinated care and essential community resources.


What Is ECM/CS?

Enhanced Care Management and Community Supports is a Medi-Cal program that provides whole-person, coordinated care for those who need it most. It’s tailored to Medi-Cal members with a Managed Care Plan who meet criteria for specific populations of focus, including:

  • Children with mental health needs, substance use disorder, or those involved in the Child Welfare system
  • Pregnant and post-partum adults, especially those enduring racial and social inequality
  • Families experiencing homelessness

Services Offered

A personalized care team works with each individual or family to address their unique needs. Services may include:

  • Personalized care coordination
  • Assistance with housing, transportation, and family needs
  • Physical and mental health support
  • Connections to community services and resources

The care team meets individuals where they are—whether at home, in the community, or at the school-based health center.


How to Access the Clinic

The clinic is open to walk-ins every Friday, or community members can call ahead to schedule an appointment.
